Care 4 U Clinic's Dr. Renee Murphy is available to assist you and your family with basic medical care needs.  She is available by appointment Monday - Friday between the hours of 9 a.m. and 4:30 p.m.  To fully meet the needs of her rural patients, Dr. Murphy also makes herself available for home vists, emergencies and after hours care.

Fee Schedule

Limited Office Visit:
$50 Brief visit up to 15 minutes.

Basic Visit:
$75 - Basic visit up to 30 minutes.

Physicals and Extended Visits:
$100 - Up to 60 minutes.

Additional Charges

After Hours and Home Visits:
$10 fee added to the above charges.

Splinting, Casting, Suturing, or                    Nebulizer Treatments:                                            Additional charge  for supplies.

Lab Draw or Injection Only:
$10 fee.  Please note some tests must be  processed at an outside lab. The patient will receive an additional charge directly from the lab. 

Cash payment is due at time of service. If you have a high deductible insurance, a MedFlex plan or a Health Savings Plan, we will be happy to provide you with receipts for your claims.
